DM Orthotics Customer Feedback Prize Draw – Terms & Conditions
Promoter: DM Orthotics Ltd, 2 Cardrew Way, Cardrew Industrial Estate, Redruth, TR15 1SS.
All customers who complete and submit the DM Orthotics customer feedback form, including providing a valid name and email address, will automatically be entered into the quarterly prize draw to win £50 worth of Amazon vouchers or a £50 Amazon gift card, or local-currency equivalent where available.
1. Eligibility
1.1 The prize draw is open to DM Orthotics customers worldwide, including healthcare professionals and patients/carers, aged 18 or over.
1.2 Employees of DM Orthotics Ltd, their immediate families, and anyone professionally connected with the administration of this prize draw are not eligible to enter.
1.3 Where feedback is being provided on behalf of a customer under 18, the parent or legal guardian completing the form must be aged 18 or over and will be the person entered into the prize draw.
2. How to enter
2.1 To be automatically entered into the prize draw, participants must complete and submit the DM Orthotics customer feedback form, including providing a valid name and email address.
2.2 Only one entry per person per quarterly prize draw is permitted.
2.3 No purchase is necessary to enter the prize draw.
2.4 Entries that are incomplete, submitted without valid contact details or otherwise do not comply with these terms and conditions may be disqualified.
3. Prize draw dates and winner selection
3.1 One winner will be selected at random on or around 31 March, 30 June, 30 September and 31 December each year.
3.2 The first prize draw will take place on 31 December 2026.
3.3 Entries received before each quarterly draw date will be included in that draw. Entries received after a draw has taken place will automatically be carried forward to the next quarterly draw.
3.4 The winner will be selected at random from all eligible entries received for the relevant draw.
3.5 The draw will be conducted using a random selection process.
3.6 The winner will be contacted by email within 7 working days of the relevant draw.
3.7 If the winner does not respond within 14 days of being contacted, DM Orthotics reserves the right to select an alternative winner at random.
4. The prize
4.1 The winner will receive a £50 Amazon gift card, or local-currency equivalent where available.
4.2 The prize is non-transferable and there is no cash alternative.
4.3 The prize will be supplied in a form appropriate to the winner’s country of residence, subject to Amazon gift-card availability in that country.
4.4 DM Orthotics reserves the right to substitute the prize with an alternative prize of equal or greater value if circumstances outside our reasonable control make this necessary.
5. Winner announcement
5.1 The winner’s first name and surname initial may be published on the DM Orthotics website or social media channels to indicate that a valid prize award has taken place.
5.2 If a winner does not wish their name to be published, they may contact DM Orthotics to request that it is not published.
6. Feedback and marketing
6.1 Feedback submitted through the form may be used by DM Orthotics to help improve its products and services.
6.2 Where a participant separately gives permission for their comments to be used for marketing purposes, DM Orthotics may use those comments in its marketing materials, website, social media and other promotional communications.
6.3 Agreeing to the use of feedback for marketing purposes is not a condition of entry into the prize draw.
7. Personal information
7.1 Personal information provided in connection with the prize draw will be used for the purposes of administering the prize draw, contacting the winner and fulfilling the prize.
7.2 Personal information will be handled in accordance with the DM Orthotics Privacy Policy.
7.3 Entering the prize draw does not automatically subscribe participants to DM Orthotics’ marketing communications. Any marketing communications will be subject to the participant’s separate marketing preferences and applicable data protection requirements.
8. General
8.1 DM Orthotics reserves the right to refuse or disqualify any entry that it reasonably believes does not comply with these terms and conditions.
8.2 DM Orthotics reserves the right to amend, suspend or withdraw the prize draw where circumstances outside its reasonable control make this necessary.
8.3 DM Orthotics’ decision regarding the administration of the prize draw and selection of the winner is final.
8.4 These terms and conditions are governed by the laws of England and Wales.
